Usage Note
exprimer is used both transitively (exprimer une idée) and reflexively (s'exprimer = 'to express oneself' or 'to speak'). The reflexive s'exprimer is very common in formal and educational contexts — a teacher may say exprimez-vous clairement.

Natural Translation
He has trouble expressing himself in public.
Literal Translation
He has some-difficulty to oneself-express in public.
